Your Visit To Your Family Doctor
What To Bring With You
You will need to bring your insurance card and a photo ID. If you have any previous
medical records please bring them. Dr. Sherman will need a list of all your medications
including the dose and the times you take them.
Alternatively, you may want to just bring them with you. Make a list of all your
concerns so that you may discuss them with Dr. Sherman.
Your Registration Process
Please plan to arrive at least 15 minutes early to complete your registration.
You may want to down load our registration forms, complete them and bring them
with you. They are also available in our office. You will be asked to sign our
HIPAA form, as required by law, so that you are aware of our privacy policies.
Your Payment
Be prepared to pay your deductibles, co-pays, co-insurance and any other amounts
due at the time of your visit. Please remember that your appointment time is
valuable. If you are unable to make your appointment please let us know at least
24 hours ahead of time so that we may schedule someone else that needs care.
If you do not, you will be charged a "No Show" fee. You will be called
the day before to confirm your appointment. Lastly, please remember if Dr. Sherman
is running late, he may be handling an emergency or complex patient problem.
During your visit Dr. Sherman will give you the same dedicated attention.
